Останавливают ли кераринг прогрессию кератоконуса?

На этот счет имеются различные мнения. На наш взгляд, кераринг не останавливает прогрессию кератоконуса. Мы неоднократно оперировали пациентов с кераринг методом сквозной кератопластики (СКП).

Как долго может длиться действие кераринг?

У каждого пациента по разному. Есть пациенты, которые носят кераринг несколько лет и имеют стабильную остроту зрения, однако есть и такие, которые приходят на СКП через год после имплантации колец.

Сколько максимально могут скоррригировать кераринг?

По сферическому компоненту рефракции 11 Диоптрий, по цилиндрическому – 5 Диоптрий.

Не легче ли сразу сделать СКП?

Каждому методу – свое время. Если у пациента нет абсолютных показаний для СКП, следует предпочесть более деликатную хирургию, каковой является имплантация кераринг. Возможно, пациент будет иметь высокую остроту зрения и стабильный кератоконус в течение нескольких лет, что даст возможность отсрочить проведения такой сложной операции, как СКП.

Можно ли сочетать кераринг с другими методами лечения, и с какими именно?

Можно. Прежде всего – с кросслинкингом.(подробнее сайт crosslinking.ru). Таким образом, достигаются две цели – исправление остроты зрения (кераринг) и остановка прогрессирования кератоконуса (кросслинкинг).

Не осложнит ли имплантация кераринг последующей кератопластики?

Нет, в большинстве случаев не осложнит.

Будет ли видно кольца невооруженным глазом?

Нет, их сможет увидеть офтальмолог при осмотре в щелевую лампу.

Как проводится операция?

Операция имплантации кераринг проводится под местным или общим (по желанию больного) наркозом. Операция амбулаторная. Пациент приходит на осмотр через три дня после операции.

What is the Keraring ?

The Keraring is an orthesis composed of two 5mm-diameter semicircular segments with variable thicknesses. The ring is made of Perspex CQ Acrylic, which is the same material used for more than 50 years in manufacturing intraocular lenses.

The ring is perfectly tolerated by the organism and there is no risk of rejection. It is the most modern and safest treatment for keratoconus.

What is the Keraring indicated for?

  1. Moderate to high myopias
  2. High myopic astigmatism, simple or compound
  3. Irregular astigmatism: In keratoconus and after corneal transplants

The Keraring implant is mostly indicated for keratoconic patients of any age with an evolving condition and intolerance to contact lenses or with sharp distortions in the cornea shape, which usually occur after transplants.

How is the surgery like?

The surgery is performed in an operating room with all precautions concerning asepsis, that is, the process of cleaning to remove pathogenic microorganisms. Anesthetic eyedrops are used. After the targeted area is cleaned, which is the area comprising the eyes, nose and forehead, the patient’s face is covered by sterile surgical drapes, aiming at protecting the surgical field and prevent contamination. The procedure is totally painless and takes about 10 minutes

The use of antibiotic eyedrops and anti-inflammatories provides a more comfortable and safer post-operatory.

In three days, the patient is able to return to his/her normal activities.

What are the risks involved in the surgery?

The risks are low. As in any surgery, infections can occur. Other complications inlclude implant extrusion or migration In that case, the ring will have to be removed but may be reimplanted at a later date. There is no risk of rejection. The surgery does not prevent nor hinder an eventual cornea transplant. When the ring is removed the corneal reverts to its preoperative shape and condition.

How is the post-op recovery like?

Visual rehabilitation is quick. The day following the surgery, the patient’s vision shows improvement, and it stabilizes after the third month. During the recovery period, it is normal to experience some fluctuation in vision. Generally, in the mornings, the patient’s vision is good, however, along the rest of the day his/her vision may be slightly blurred. Once it is not an esthetical surgery, it may be necessary the use of eyeglasses or contact lenses to complement the visual correction. Nonetheless, the visual acuity may be good, even though there are some residual diopters.

Preparation for surgery

There are no diet restrictions on the day scheduled for surgery. It is advisable that the patient takes a bath and washes his/her hair.

He/she should arrive at the clinic 30 minutes before surgery time. Before entering the operating room, the patient must change his/her clothes and put on an appropriate garment

Eyedrops will be administered at regular intervals until surgery time.

Post-Op Instructions

1. Normal symptoms immediately after surgery:

You may experience some pain, photophobia (aversion to light), teariness and/or redness in the eye during the first few days. However, the late appearance of those symptoms is abnormal and you should communicate them to your doctor. In case of doubt, you should contact the clinic.

2. Medications used after surgery

Eyedrops Voltaren 1 drop ever 15 minutes during the first 3 hours after surgery
Oflox 1 drop every 1 hour from the 4th hour after surgery until 11 p.m. 1 drop every 4 hours until 11 p.m. 1 drop every 8 hours
Maxitrol 1 drop every 6 hours until 7 p.m.

Important: It should be observed a minimum interval of 15 minutes between the administration of this medication and the other ones.

Recommendations:

You will need:

  1. Micropore adhesive tape to hold the eye shields in place.
  2. Sterile gauze and saline solution for eye asepsis.
  3. Soapex liquid soap for hands and eye shields asepsis.

3. Precautions to prevent contamination

During 30 days, you should observe certain procedures to prevent contamination.

You must avoid:

  1. immersion baths (in swimming pools, beaches, bathtub);
  2. saunas;
  3. heavy physical exercises;
  4. rubbing the eyes;
  5. eye make-up.

During the first 10 days after surgery, you must shower from the neck below and have someone washing your hair, taking care not to let water into your eyes.

The eye asepsis is done using saline solution and sterile gauze, in a very delicate way, to avoid any trauma.

It is important that you wash your hands very well with Soapex liquid soap and dry them with disposable towels before cleaning your face and using the eyedrops.

Avoid going to busy and crowded places during the first 30 days.
